MINIMUM EDUCATION & EXPERIENCE
  • Bachelor's degree in Social Work, Psychology, or related field
  • Minimum 1 year of experience working with chronically mentally ill, dually diagnosed or emotionally disturbed adults
PREFERRED EDUCATION & EXPERIENCE
  • Master's degree in Social Work, Psychology, or related field
  • 2 years of experience working with chronically mentally ill, dually diagnosed or emotionally disturbed adults
  • Electronic Health Records (EHR) experience
KNOWLEDGE, SKILLS, & COMPETENCIES
  • Knowledge of and ability to interpret Backer Act, Marchman Act, Client Rights & Abuse reporting requirements
  • Advanced organizational and time management skills
  • Capability of interpreting and adhering to center policies & procedures
  • Requires minimal supervision regarding use of time and proficient in prioritizing work assignments
  • Skilled at defining problems, collecting data, establishing facts, & drawing valid conclusions
  • Aptitude to abide by a code of ethics and practice standards
  • Must possess good clinical skills to assess the needs of clients
  • Advanced substance use/abuse knowledge
  • Familiarity with DSM-IV & GAF
  • Case management experience
  • Ability to communicate effectively with professionals, staff members, clients, & families
  • Proven track record of working well with others as part of a team
  • Competence completing required paperwork accurately & legibly
  • Computer literate 
RESPONSIBILITIES
  • Evaluate client & family needs
  • Attend & participate in multi-disciplinary treatment teams, weekly
  • Develop appropriate discharge plans & recommendations to link clients with appropriate community resources & document same in coordination with assigned catchment areas
  • Document discharge goals on treatment plans
  • Coordinate with nursing, medical, & D/C staff on clients care & discharge plans
  • Conduct weekly discussion groups for clients
  • Maintain client records & document all contact with clients, family and/or supports
  • Record all assessments and other clinical events in progress notes
  • Attend & participate in staff development and in-service trainings
  • Assist the program supervisor with training of practicum students
  • Performs all other duties as assigned
