PURPOSE
KNOWLEDGE, SKILLS & COMPETENCIES
Face-to-face assessment interviews with clients to determine their level of substance abuse needs.
Accompany clients to & links them with community resources, as needed.
Counsels clients and family members, when appropriate, on relevant clinical issues.
Continuous outreach to all clients in caseload.
Advocate for clients, such as assisting with locating treatment facilities or halfway houses
Complete applications for services and discuss client needs with their provider(s).
Coordinate provider care i.e. psychiatrists, therapists, medication clinics, and external services such as hospitals, social service agencies, and/or housing agencies.
Monitors the treatment process to ensure required services are provided in a timely manner.
Attend staff meetings.
Documentation including progress notes, progressive reviews, and other reports, on multiple platforms.
Complete an extensive monthly report in order to convey data to DCF.
Conduct assessments, as instructed, as part of client's medical records.
Attain locus assessment upon admission and every six (6) months, as needed, to determine level of care.
Daily entries into Credible (EHR) schedule and COC module.
On-call: To assist in responding to unscheduled services for clients whose assigned care coordinator is unavailable at the time.

Manages a case load of no more than 15 adults who have been identified as high utilizers in the community, which includes a collaboration with southeast Florida behavioral health network. Such high utilizers suffer with severe and persistent mental illness, along with substance use/abuse related barriers. The goal is to offer linkages, referrals and any other means that can aid in the identified person’s ability to be independent in the community.
MINIMUM, EDUCATION & EXPERIENCE
PREFERRED EDUCATION & EXPERIENCE
MA/MS in Social Sciences or related field.
2 years of case management experience working in the behavioral health industry.
Experience working with individuals facing barriers due to substance use/abuse.MINIMUM, EDUCATION & EXPERIENCE
- BA/BS in Social Sciences or related field.
- 1 year of experience working in the behavioral health industry.
- Valid FL Driver's License.
PREFERRED EDUCATION & EXPERIENCE
MA/MS in Social Sciences or related field.
2 years of case management experience working in the behavioral health industry.
KNOWLEDGE, SKILLS & COMPETENCIES
- Familiar with 65e-15 regulations, Baker Act and Marchman Act.
- Demonstrate the ability to interpret and abide by confidentiality laws, abuse reporting guidelines, and client rights requirements as well as operate by a code of ethical and practice standards.
- Basic assessment and case management skills.
- Knowledge of mental health diagnosis and behaviors.
- Excellent communication skills, written and verbal.
- Capability of working individually as well as part of a team.
- Superb organizational and time management skills.
- Ability to educate adults on illness, medication compliance, community resources, money management & life skills.
- Knowledge of psychiatric medications.
- Must possess the ability to handle physically and/or verbally aggressive clients.
