Substance Use Care Coordinator

Position Summary
South County Mental Health Center, Inc. is seeking a Substance Use Care Coordinator to our Mobile Response Team. This role provides care coordination to assigned individuals involved with MRT crisis intervention. Substance Use Care Coordinator will be responsible for monitoring care linkages and/or following up with clients (if services are refused) for 72 hours to ensure successful care linkages both internal and external.  Ongoing care coordination provided by the coordinator will help to ensure that individuals receive timely and appropriate services and to monitor the success of referrals.  The provision of care coordination services will allow for adjustment of referrals or modification of services as necessary and strengthens continuity of care for the individuals served.

Minimum Qualifications
  • Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, or a related human services field.
  • Two years full-time or equivalent experience working with adults and children at risk
  • Must have a valid Florida Driver’s License.

Preferred Qualifications:
  • Enrolled in a Master’s Program in a related field and have professional experience in substance use. 

Duties and Responsibilities
  • Face-to-face assessment interviews with clients to determine their level of substance abuse needs.
  • Accompany clients to & links them with community resources, as needed.
  • Counsel clients and family members, when appropriate, on relevant clinical issues.
  • Continuous outreach to all clients in caseload.
  • Advocate for clients, such as assisting with locating treatment facilities or halfway houses
  • Complete applications for services and discuss client needs with their provider(s).
  • Coordinate provider care i.e. psychiatrists, therapists, medication clinics, and external services such as hospitals,
  • social service agencies, and/or housing agencies.
  • Monitors the treatment process to ensure required services are provided in a timely manner.
  • Attend staff and community meetings.
  • Documentation including progress notes, progressive reviews, and other reports, on multiple platforms.
  • Complete an extensive monthly report to convey data to DCF.
  • Conduct assessments, as instructed, as part of client's medical records.
  • Daily entries into Credible (EHR) schedule and COC module.


Physical Demands
  • Must be in good physical condition with no restrictions of movement. 
  • Must be able to apply Therapeutic Effective Aggression Management (CPI/CARE) Techniques without limitations.

  • May require participation in audits, inspections, or urgent compliance matters

  • Collaboration with leadership and department teams
  • Organizational readiness for surveys and inspections
